Project MyCare

Funded by a grant from the Blue Cross Blue Shield of Massachussetts Foundation


Steppingstone is sharing in the responsibility of containing healthcare costs by providing quality care through developing and incorporating project MyCare into programming at Steppingstone. MyCare project offers assistance to those diagnosed with chronic illness so they are better prepared to manage their illness. This is accomplished by providing medical case management, nutritional counseling, and health education. Outcomes of these services are an improved quality of care that is value-based as clients maintain and improve their health outcomes.

Steppingstone is partnering with Stanley Street Treatment and Resources (SSTAR) Community Health Center, who provides a Registered Nurse Case Manager, for guiding clients in selecting appropriate and preventive care. SSTAR's nutritionist provides one-on-one nutritional counseling to clients. While education in managing chronic illness is provided by client workshops based on the Chronic Disease Self Management program developed by Standford Medical School.



Steppingstone, a licensed provider of Standford School of Medicine Chronic Disease Self Management Program will be offering ongoing client workshops to those diagnosed with chronic illness.


Specific services provided by the MyCare project include:

  • On-site medical case management and intervention
  • Nutritional counseling and education
  • Prevention and health promotion activities, including a preventive care schedule
  • Chronic disease self management workshops
  • Guidance with obtaining appropriate level of care
