Coordinated Care Network

About
Steppingstone is partnered with the Coordinated Care Network (CCN), which is a designated MassHealth Behavioral Health Community Partner (BHCP). The Coordinated Care Networks works directly with the Accountable Care Organizations and Managed Care Organizations to integrate care and improve health outcomes for MassHealth members with complex long-term and/or behavioral health needs.
Our team of care coordinators work to help MassHealth members connect to appropriate and coordinated care with primary care physicians, medical specialists, Accountable Care Organizations (ACOs), and Managed Care Organizations (MCOs). CCN helps ensure members have access to medical, behavioral health, social supports, and/or long term support services that they need and want. We also provide support to address social needs that affect people, including housing, nutrition, employment, and wellness coaching to achieve goals for better health.
The Coordinated Care Network (CCN) is comprised of six agencies called Affiliated Partners (Highpoint Treatment Center, Bay State Community Services, Brockton Area Multi Services, Child & Family Services, Duffy Health Center, and Steppingstone) and two Material Subcontractors: Brockton Neighborhood Health Center and Community Health Center of Cape Cod.

Admission Criteria
- Must be a MassHealth member
- Must be an adult between the ages of 18 and 65
- Eligibility for the CP program includes members diagnosed with complex mental illness and/or substance use disorders, but eligibility is not limited to those specific populations.
